Treatment Room
We make sure all our new patients feel welcome, safe, and taken care of. During your first visit, a doctor will review your health history. For diagnosis, they will complete an examination and use any other special procedures (such as x-ray) to understand your issues and address any concerns. From there, the doctor will explain any results and help you understand treatment options. We typically proceed with care the same day.
For a shorter time in the waiting room, please click on, print, and fill out both pages of the New Patient Intake Form: Page #1 and Page #2 then just bring both pages with you to your first appointment. These forms are also available in the office.
